CHECKS OR MONEY ORDERS ACCEPTED | Fax Form
1) After filling your cart, at the first check out page select "Mail-In" or "Call-in" as form of payment. At the final checkout page, please enter your shipping and billing information, including email address (If you don't have an email just enter "none" and Print the form). Click "submit." Your order will be sent through a secure connection to our office, while you receive a copy (if you entered a valid email address). The e-mail receipt you receive should have the following information:
1. Your name and address
2. Product titles with item numbers
3. Total, including shipping.
2) Then mail or fax a printed copy of your order or email receipt, along with a Personal Check or Money Order to:
